Hospice at Home East Hampshire
What is Hospice at Home?
Hospice at Home is provided by a team of registered nurses and health care assistants in East Hampshire, experienced in palliative care and bereavement counselling. They are supported by admin, finance and counselling teams.
In addition, and an integral part of the Rosemary Foundation, are our band of volunteers who support us in admin, finance, reception, fundraising and visiting patients.
The hospice at home service, which is free of charge, supports adults over the age of eighteen who have life-limiting conditions and are approaching the end of their life.
Patients who are referred to us are assessed by one of our nurses, who will visit the patient and their family, and also contact other healthcare professionals involved, particularly the GP, and District Nurse, to assess the type and degree of care required and how this may best be provided.
Each patient in the hospice at home service has a named nurse but all the team are involved as needed, working in pairs to enable support for the patient and their carers.
Anyone may refer a patient to us but normally it is not possible to take on the care of any patient without the agreement of first their GP.
We are a charitable organisation relying entirely on donations, allowing us to focus entirely on the care we provide to your loved ones.
